• Swim School Office has received 24 hours notice prior to the lessons commencement.
• Student has not exceeded their maximum of two makeup lessons per term.
• Any notice less than 24 hours will only be accepted with current and relevant medical documentation, covering ALL missed lessons.
• Make up lessons will only be taken within the enrolled term where lessons were missed and only where there is a position vacant.  We cannot guarantee times will be available to suit your needs.
• Makeup lessons can be booked from week 3 of term.

The SOPAC Swim School understands there are times when your child will be unable to attend swimming lessons for a variety of reasons.  As of Term 1 2010, during Term 1 and Term 4 we do not require a phone call if you can’t make the lesson (excluding special needs and private lessons).  If however your child misses consecutive lessons due to illness, you may apply for a credit.

CANCELLATION POLICY

Credits will only be issued under the following guidelines:

• Credits are only considered for students who have incurred the following:
         o  Major Illnesses
         o  Hospitalisation
         o  Moving house or relocation
• Credits will not be granted for any other reason.  This includes absences due to going away on holidays, exams or clashes with other activities.
• All applications must be received prior to the completion of the enrolled term.
• Credits will only be considered for 3 or more consecutive absences from lessons.
• Credit – money will be credited to you to use for future swimming lessons and will remain on your file for 12 Months.
  
  To apply for credit:
  An application for credit form must be completed along with below documentation.
           o   Letter outlining the reason for application and number of lessons missed (To:Aquatic Supervisor).
           o    Doctors Certificate 
           o    Receipt
           o    Swim School Entry Pass

Once your credit has been approved, a letter will be sent outlining the amount of credit the swim school is holding.  All approval for applications for credit is at the discretion of the Aquatic Supervisor.

Please note:  In the event of missed or cancelled lessons, refunds will not be granted.

LANE AND TEACHER CHANGES

We endeavour to provide you with the most consistent experience as possible, however there are occasions when lane and teacher changes are required.  Please be advised that we cannot guarantee instructors from term to term.  By law all instructors working at the Sydney Olympic Park Aquatic Centre are fully qualified in swim teaching, and undergo regular training days to improve and maintain their high level of teaching standards.  Please note refunds will not be given as a result of lane or teacher changes.

LESSON CANCELLATIONS

Class occupancy is a major factor in determining whether or not a class will need to be cancelled.   In the event that your chosen class is not 60% full, a Swim School staff member may contact you to transfer to an alternate class.  All lesson cancellations are at the discretion of the Aquatics Supervisor.

RE-ENROLMENT PROCEDURES

The SOPAC Swim School has set enrolment procedures to assist you with attaining your preferred day and time for swimming lessons term by term.  Please be advised, that each child’s enrolment for the following term can be guaranteed for same day, time and level bookings between weeks 3- 6 of term.  Outside this period your child’s position in program cannot be guaranteed.  For further information regarding enrolment please speak to the Swim School Office or visit our website.

To guarantee your enrolment a minimum deposit of 50% is required for all re-enrolments. All remaining balances must be finalised by the end of Week 2 of every term. Any students with money owing after this week will be removed from program. Please note, Swim School entry cards will not be issued until full payment has been received.

SYDNEY OLYMPIC PARK AQUATIC CENTRE ENTRY

After full payment for swimming lessons you will receive your Swim School Entry Card.  Please present this pass at the Service Desk to gain entry for you lessons.  Please note the entry card permits access to student and on parent/carer (Additional parent/carer will be charged a spectator fee).  If you do not have your card you will be charged the full admission fee.  Lost, stolen or damaged cards are the responsibility of the cardholder and a $5.50 fee will be charged for a replacement card.

By signing the bottom I agree to use the services provided by the Sydney Olympic Park Aquatic Centre and Swim School as permitted by these T&C mentioned above.  

In particular I understand and agree to:

The makeup policy,that during Summer terms 4 and 1 there will be no makeup lessons permitted,and outside these terms I will provide adequate notice and/or the appropriate paperwork when requesting a replacement lesson.  I am fully aware that there will be a maximum of 2 makeup lessons per term, and that SOPAC Swim School cannot guarantee that there will be available makeup lessons to replace the lessons missed;That no refunds will be offered for missed lessons or cancellation of the term;

1. That credits for missed lessons will only be approved if the appropriate criteria are met and the required documentation is provided to the Swim School Office. I am fully aware that all approval for applications of credit is at the discretion of the Aquatics Supervisor. And there is no guarantee of a future preferred position in lessons;
2. That teachers and lanes are subject to change;
3. I am fully aware that if the enrolment numbers of the class are below the required 60% occupancy it is the discretion of the Aquatics Supervisor whether the class is cancelled and the current class re-scheduled to an alternate convenient day or time;
4. I am fully aware that if I do not re-enrol with in the specified period of weeks 3 – 6 that SOPAC Swim School cannot guarantee me an enrolment in their program; and
5. The Swim School Term and Conditions on the reverse of my entry card.
6. The above Terms and Conditions will apply until further notice. Any changes to the T&C will be notified in writing during the next enrolment period.
